Acquired Brain Injury Waiver and Money Follws the Person Program
THE ACQUIRED BRAIN INJURY (ABI) WAIVER AND MONEY FOLLOWS THE PERSON PROGRAM PROVIDE NON-MEDICAL HOME AND COMMUNITY-BASED SERVICES THAT ENABLE PEOPLE WITH AN ACQUIRED BRAIN INJURY TO ACHIEVE THEIR HIGHEST POTENTIAL AND LIVE IN THE COMMUNITY INSTEAD OF INSTITUTIONS AND NURSING HOMES.
We help brain injury survivors live their lives to the fullest, in the least intrusive and restrictive environments possible, of their choosing. We focus on person-centered goals, activities, work, community and social inclusiveness. The goal of Cortex, LLC is to enhance lives of brain injury survivors and help and support families and loved ones.
Brain Injury is different for each individual, developing an ABI Waiver Program or Money Follows the Person Program with services that are specific to the individual needs of each person is done as a person-centered collaborative effort with the survivor, family, loved ones and/or conservator, doctors, social worker, clinical staff and others as needed. ABI Waiver services can range from a few hours a day, to 24 hours a day, depending on the needs of the individual.
